Ten Seneca County food service operations and retail food establishments received critical citations during inspections conducted by the Seneca County General Health District in June 2026, according to records from the district’s public food safety inspection database.
Most of the critical violations were marked corrected during the inspection. At four facilities—a Tiffin Kroger, The Corner Restaurant in Bloomville, a Tiffin Bob Evans and Mi Tequilas in Fostoria—at least one critical violation was not recorded as corrected on-site. Under Ohio’s food code, a critical violation is one directly linked to the risk of foodborne illness.
Kroger #594 — 790 W. Market St., Tiffin
Inspected Thursday, June 11, 2026, the Tiffin supermarket received one critical violation during a standard/process review inspection. Health district inspectors cited the store under the cold-holding rule for time/temperature controlled for safety (TCS) foods after a seafood island case registered 50°F. Inspectors recorded shrimp at 50°F and salmon at 45°F, above the 41°F maximum required to limit bacterial growth. According to the record, no temperature alert had been received for the case, and staff planned to monitor whether it was in defrost mode and remove the shrimp and salmon if it was not. Kroger #594 carries a Risk Level IV designation, the highest risk category assigned to food operations.
Old Fort Country Market — 8080 Main St., Old Fort
The Old Fort market received two critical violations during a standard/process review inspection Tuesday, June 16, 2026. Health district inspectors cited a contamination violation after observing potatoes stored less than six inches off the ground, and a cold-holding violation after food in the front of the deli cooler was found above the maximum safe temperature. Food held toward the back of the cooler was within range. The out-of-temperature food was discarded and a service call was placed to repair the cooler. Both violations were corrected during the inspection. The facility carries a Risk Level IV designation.
The Corner Restaurant — 7980 U.S. 224, Bloomville
The Bloomville restaurant received one critical violation during a Friday, June 5, 2026, inspection. Health district inspectors cited a cold-holding violation, noting the pie cooler was not holding at 41°F and directing the operator to inspect the cooler to ensure proper operation. A non-critical, repeat violation cited a food packaged in the facility that was not labeled; inspectors directed staff to label containers of take-home salads. The facility carries a Risk Level IV designation.
The Front Nine — 6730 N. Township Rd. 69, Kansas
The Front Nine received two critical violations during a standard/critical control point inspection Tuesday, June 16, 2026. Health district inspectors cited uncovered food in the freezer as a failure to properly protect food from contamination by separation, packaging and segregation. A second critical violation—a repeat—cited ready-to-eat, TCS food not properly discarded when required, with out-of-date food observed in the cooler. Both violations were corrected during the inspection. The facility carries a Risk Level IV designation.
GW’s — 275 S. Washington St., Tiffin
The Tiffin establishment received two critical and one non-critical violation during a Monday, June 1, 2026, inspection. Health district inspectors cited a repeat contamination violation after observing uncovered food in the cooler and freezer, and a date-marking violation after observing unmarked refrigerated, ready-to-eat TCS food in the cooler. Both critical violations were corrected during the inspection. A non-critical violation cited debris on the ice maker. The facility carries a Risk Level IV designation.
Fatheads — 218 Washington St., Republic
The Republic restaurant received two critical and one non-critical violation during a follow-up inspection Thursday, June 18, 2026. Health district inspectors cited uncovered food on a counter and in the freezer as a contamination violation, and ready-to-eat, TCS food held in the cooler beyond the seven-day safety range as a disposition violation. Both critical violations were corrected during the inspection. A non-critical violation cited handwashing sinks not conveniently located as required, with no hand sinks present in the prep or kitchen areas. Inspectors noted that a mop sink had been installed since the facility’s previous inspection but that hand sinks still needed to be installed to meet food code standards. Non-critical violations cited a box of bananas stored on the floor of the dry storage room and areas needing cleaning, including the wall under the microwaves in the prep sink and fryer areas.
